Form 1: New Customer Agreement and Information Form (first time customers only)
Sign our new customer agreement, and fill in information about your medical history.
Form 2: Canadian Medication Order and Re-Order Form
Fill in your contact information and your preferred method of payment. Indicate the medications you wish to order with the dosage and price.

Form 3: Prescription fax cover sheet
Attach a copy of your prescription to the fax cover sheet where indicated (using scotch tape is preferable). For multiple prescriptions, print extra fax cover pages. Please attach one prescription per fax cover sheet.
