| Once you have filled your shopping cart, click on the Check Out button, and complete the information requested. You will receive an immediate email confirmation of your online order. You may also print a copy of the completed order form for your records. |

Print the Prescription Fax Cover Sheet, attach your doctor's prescription to the sheet where indicated (use scotch tape). Important: Please make sure that you include your online order number on the Fax Cover Sheet where indicated. For multiple prescriptions, print extra fax cover pages. Fax your prescription to our toll-free fax number 1-866-290-2272. We will process your order and email you an order confirmation. Please allow three days for processing before contacting us to follow up on your order. |
