Why is Epclusa prescribed?

Epclusa (Sofosbuvir/ Velpatasvir) is an antiviral medication used in combination with other drugs to treat hepatitis C. It is not effective for treating the flu, colds, or other viral infections.

What does it do?

Epclusa is a combination antiviral treatment containing two active ingredients that work through different mechanisms. Sofosbuvir inhibits an enzyme called NS5B RNA polymerase, preventing the hepatitis C virus from replicating. Velpatasvir targets and blocks the NS5A protein, which is also necessary for the virus to reproduce.

How should I take it?

Swallow the tablet whole. Do not crush, split, or chew it. Take one tablet of Epclusa once daily, with or without food.

What if I missed a dose?

If you miss a dose, take it as soon as you remember. However, if it's nearly time for your next dose, skip the missed dose. Do not double dose.

What are the special precautions before taking Epclusa?

Inform your doctor about all the medications you are taking, your current and previous medical history.
  • Diabetes
  • Organ transplant
  • Liver diseases
  • Heart disease
  • HIV or AIDS

Please also note the following precautions

  • If you have a history of hepatitis B, consult your healthcare provider, as this medication may reactivate the hepatitis B virus.
  • If you're taking this medication with ribavirin, note that ribavirin can cause serious birth defects. Talk to your healthcare provider if you or your partner are pregnant or planning to become pregnant.
  • This medication may affect your blood sugar levels. If you have diabetes, ask your healthcare provider if adjustments to your diet or medications are necessary.

Does it have any side effects?

Is it safe to take it with another drug?

Inform your healthcare provider about all the medications you are taking, including prescription drugs, over-the-counter medications, vitamins, and herbal supplements. Be sure to mention the following:
  • Amiodarone. Seek immediate medical help if you experience very slow heartbeats, chest pain, extreme fatigue, or lightheadedness (feeling like you might faint).
  • Omeprazole (Prilosec) or any antacids. Wait at least 4 hours after taking Sofosbuvir and Velpatasvir before using them.
  • P-gp inducers or moderate to strong CYP inducers (such as St. John’s wort, Rifampin, or Carbamazepine). These may lower the effectiveness of Sofosbuvir and Velpatasvir. Combining them is not recommended.

The clearance of HCV infection with direct-acting antivirals can affect liver function, which may influence the safety and effectiveness of other medications. Frequent lab monitoring and potential dose adjustments may be needed.

Keep a list of all your medications to share with your healthcare provider and pharmacist. You can also ask them for a list of medicines that interact with Sofosbuvir/ Velpatasvir. Always consult your healthcare provider before starting any new medication to ensure it is safe to take alongside Sofosbuvir and Velpatasvir.

Is it safe to take it with another drug?

Epclusa should be stored at room temperature, below 86°F (30°C), in their original container. Do not use the medication if the tamper-evident seal on the carton or the packet is broken or damaged.

What action should I take in case of overdose?

If you take an overdose of Epclusa, contact your healthcare provider immediately, go to the nearest emergency room.

Is it safe for pregnant or breastfeeding women?

Pregnancy: It is unclear whether Epclusa can affect pregnancy or harm an unborn baby. Inform your healthcare provider if you are pregnant or planning to become pregnant.

Breastfeeding: It is not known if Epclusa passes into breast milk. Let your healthcare provider know if you are breastfeeding or intend to breastfeed, and they will advise you on whether it’s safe to take Epclusa while breastfeeding.
