Why is Tasmar prescribed?

Tasmar (Tolcapone) is used as an add-on to carbidopa/levodopa in patients with Parkinson’s disease who experience motor “wearing-off” symptoms such as stiffness, tremors, muscle spasms, or unpredictable off-periods despite optimized levodopa therapy. It’s reserved for those who haven’t achieved adequate symptom control on carbidopa/levodopa alone.

What does it do?

Tolcapone is a catechol-O-methyltransferase (COMT) inhibitor. By blocking COMT, it slows the breakdown of levodopa in the brain and periphery, extending levodopa’s half-life and maintaining steadier dopamine levels to reduce Parkinsonian symptoms.

How should I take it?

Take Tasmar exactly as prescribed by your doctor. The typical dose is a 100 mg tablet orally three times a day, always alongside levodopa/carbidopa, spacing doses approximately every 8 hours at the same times each day.

Swallow tablets whole, with or without food. If the initial dose is well tolerated but insufficient after 3 weeks, your doctor may increase it to 200 mg three times daily, but only if the benefits outweigh the increased risk of liver injury.

What if I missed a dose?

If you miss a dose of Tasmar, take it as soon as you remember. However, if your next scheduled dose is approaching, skip the missed dose and proceed with your usual dosing routine. Do not take extra or double doses to make up for the missed dose.

What are the special precautions before taking Tasmar?
