Why is Xeljanz prescribed?

Xeljanz (Tofacitinib) is an oral Janus kinase (JAK) inhibitor that can be used either on its own or in combination with other medications to treat inflammatory and autoimmune conditions, including ulcerative colitis, rheumatoid arthritis, ankylosing spondylitis, psoriatic arthritis, and polyarticular juvenile idiopathic arthritis.

What does it do?

Xeljanz is a JAK inhibitor that works within your cells to block the pathways responsible for inflammation, helping to control your overactive immune system.

How should I take it?

Xeljanz is taken as tablets, which can be taken with or without food. The typical dose is two tablets daily one in the morning and one in the evening. However, your doctor may recommend taking just one tablet per day.

What if I missed a dose?

Take Xeljanz as soon as you remember but skip the missed dose if it's nearly time for your next dose. Do not take two doses at once.

What are the special precautions before taking Xeljanz?

Inform your healthcare provider about all your health conditions and any medications, herbal products, vitamins, or supplements you are taking. This will help them assess whether Xeljanz is suitable for you.

Risk for Infection: Let your doctor know if you have factors that may increase your risk of infection, such as:

  • Current infection treatment
  • Frequent or recurring infections
  • Diabetes, HIV, chronic lung disease, or a weakened immune system
  • Tuberculosis (TB) or exposure to someone with TB
  • Living in or traveling to areas like the Ohio and Mississippi River Valleys or the Southwest U.S.
  • Hepatitis B or C, or shingles
  • Smoking: If you currently smoke or have a history of smoking, inform your provider. Smoking may increase the risk of certain cancers or heart problems when taking Xeljanz.

Other Health Conditions:Tell your doctor if you have any of the following:

  • Kidney issues or previous kidney transplant
  • Liver problems
  • Diverticulitis or stomach ulcers
  • History of cancer, heart attack, stroke, or blood clots
  • High cholesterol, blood disorders, or digestive tract narrowing
  • Vaccines:Discuss any vaccines you may need. Certain vaccines should be avoided before or during Xeljanz treatment. Inform your provider about any recent or upcoming vaccines.

Does it have any side effects?

Xeljanz (Tofacitinib) can cause side effects such as headaches, diarrhea, and nausea, which are generally mild and improve over time. It may increase the risk of infections, including throat, chest, and urinary tract infections, as well as more serious infections like shingles and cellulitis. If you develop symptoms of infection, such as sore throat, cough, or fever, contact your doctor immediately.

People taking tofacitinib should also be cautious of chickenpox or shingles, as these may be more severe, and may require treatment or discontinuation of the drug. Tofacitinib may cause mild changes in blood counts, cholesterol, or liver enzymes, which your doctor will monitor. There is also a small increased risk of blood clots, including deep vein thrombosis and pulmonary embolism, seek medical attention if you experience leg swelling or shortness of breath.

Although research hasn't confirmed it, there may be a slightly higher risk of certain cancers, especially for those with a history of cancer. Additionally, tofacitinib can cause stomach or bowel problems, particularly for those also taking NSAIDs or steroids, notify your doctor if you experience abdominal pain or changes in bowel habits.

In rare cases, tofacitinib can cause allergic reactions with symptoms like swelling, rash, or difficulty breathing. Seek immediate medical help if you experience any severe symptoms.

Is it safe to take it with another drug?

Always inform your doctor about any prescription or over-the-counter medications, herbal products, vitamins, or supplements you are using.

Specifically, discuss the use of the following before starting Xeljanz:

  • Fluconazole
  • Ketoconazole
  • Rifampin
  • Tacrolimus
  • Azathioprine
  • Cyclosporine
  • Adalimumab
  • Infliximab
  • Tocilizumab
  • Etanercept
  • Certolizumab
  • Golimumab
  • Rituximab
  • Some medicines may affect the blood levels of Xeljanz, so be sure to inform your provider about all medications you're currently taking or have recently used. Always consult with your doctor for a full list of potential drug interactions.

    How should I store it?

Store Xeljanz at room temperature, in its original bottle and carton, to protect it from light. Keep it out of the reach of children.

What action should I take in case of overdose?

If you experience an overdose of Xeljanz contact your healthcare provider or go to the nearest emergency room immediately.

Is it safe for pregnant or breastfeeding women?

If you are pregnant, it is unclear whether Xeljanz could harm an unborn baby. The benefits of treating these conditions with Xeljanz may outweigh any potential risks to the baby. Consult your doctor before taking this medication.

Do not breastfeed while taking this medication, and for at least 18 hours after your last dose (or 36 hours if you are taking the extended-release tablets). If you pump breast milk during this period, do not feed it to your baby.

Canadian Prescription Drug Store dispenses medications from our fully licensed and independently verified pharmacy partners around the world from Canada, England, Australia, New Zealand, India, and Mauritius.

Legal disclaimer: The FDA, due to the current state of their regulations, has taken the position that virtually all shipments of prescription drugs imported from a Canadian online pharmacy by a U.S. consumer will violate the law. However, many legal experts disagree with the FDA's position in this regard stating that such importation is permitted under certain federal and state laws. Several State governments and consumer rights groups actively promote the importation of medications. The FDA itself provides the public information on how to import medications safely. Furthermore, there is no record of any US individual being prosecuted by the FDA for importing non-controlled prescription drugs for their personal use.
