Why is Exforge HCT prescribed?

Exforge HCT contains the active ingredients amlodipine besilate, valsartan, and hydrochlorothiazide. It is classified as an anti-hypertensive medication. This medication is intended for the treatment of high blood pressure in patients who have already had success using the individual components of valsartan, amlodipine besilate, and hydrochlorothiazide. It serves to combine these three medications into a single tablet.

What does it do?

    The ingredients in Exforge HCT lower blood pressure through three distinct mechanisms:

  • Valsartan inhibits the action of angiotensin II, a substance in the body that constricts blood vessels and increases blood pressure. By blocking the effects of angiotensin II, your blood vessels relax, leading to a reduction in blood pressure.
  • Hydrochlorothiazide helps eliminate excess fluid from the body by increasing urine production, which contributes to lowering your blood pressure.
  • Amlodipine besilate prevents calcium from entering the cells of the heart and blood vessels. This leads to the relaxation of blood vessels, increases the blood and oxygen supply to the heart, and reduces its workload.
  • How should I take it?

    Exforge HCT can be taken either with or without food. The tablets should be swallowed whole with a glass of water, ideally at the same time each day, preferably one tablet in the morning.

    What if I missed a dose?

    Take it as soon as you remember, however, if it’s nearly time for your next dose, skip the missed dose and just take the upcoming one. Do not take a double dose to make up for the missed one.

    What are the special precautions before taking Exforge HCT?

    Inform your doctor about all the medications you are taking, your current and previous medical history. Do not take Exforge HCT if:

  • Allergic to valsartan, amlodipine besilate, hydrochlorothiazide, or any other ingredients in Exforge HCT
  • You have a severe kidney disease or are undergoing dialysis.
  • You have cholestasis, a condition characterized by reduced or halted bile flow.
  • You have Anuria, a condition in which you are unable to urinate.
  • You have elevated uric acid levels in the blood, which can lead to gout.
  • You have low levels of potassium or sodium, or high levels of calcium in your blood.
  • You are taking other blood pressure-lowering medications that contain Aliskiren and have type 2 diabetes.
  • Consult your doctor if you have any additional medical conditions, such as:

  • Kidney issues or have received a kidney transplant.
  • Liver problems
  • Heart conditions, including obstructed blood flow through the heart due to narrowed valves (stenosis) or hypertrophic obstructive cardiomyopathy (HOCM).
  • Primary hyperaldosteronism (Conn's syndrome), a hormonal disorder that leads to fluid retention in the body.
  • Swelling, primarily in the face and throat, while using other medications (including an ACE inhibitor or aliskiren).
  • A diet low in salt
  • Breathing or lung issues (including inflammation or fluid accumulation in the lungs) experienced after using hydrochlorothiazide in the past.
  • Does it have any side effects?

    The most common side effects of Exforge HCT are listed below. Inform your doctor if you experience any of these side effects that concern you.

  • Dizziness
  • Headache
  • Indigestion
  • Muscle spasms
  • Back pain
  • Nausea
  • Infection of the nose or throat, sore throat, or symptoms of a common cold
    • Serious side effects may include:
  • Decreased Blood Pressure (Hypotension): this medication may lead to low blood pressure in individuals with low fluid or salt levels. This is particularly common in those taking medications that increase urination (diuretics).
  • Chest Pain or Heart Attack: when you begin taking Exforge HCT or increase your dosage, your chest pain (angina) may worsen, or you may experience a heart attack.
  • Kidney Damage: can occur while taking Exforge HCT.
  • Electrolyte Changes: may lead to changes in the levels of specific minerals in your body known as electrolytes.
  • Short-Sightedness and Glaucoma: may elevate pressure in the eye and cause or exacerbate narrow-angle glaucoma, potentially resulting in blindness.
  • Contact your doctor immediately if any of these serious side effects occur to prevent further complications or fatalities.

    Is it safe to take it with another drug?

    Some medications may interact with Exforge HCT and influence its effectiveness. You might need to adjust the dosages of your current medications or switch to different ones.

    These medications include:
  • Amphotericin, a medication used to treat fungal infections.
  • Diuretics (fluid tablets), ACE inhibitors, Aliskiren, and beta-blockers.
  • Simvastatin
  • Certain antibiotics (such as rifamycins), anti-rejection medications (like cyclosporine), and antiretrovirals (such as ritonavir) that may enhance the effects of valsartan.
  • Anti-inflammatory medications like Celebrex, and Indocid (NSAIDs), or selective cyclooxygenase-2 inhibitors (Cox-2 inhibitors).
  • Steroid medications like Cortisone, Prednisone
  • Lithium, antidepressants, antipsychotics, and medications used to manage mood swings and certain types of depression.
  • Penicillin
  • Insulin and oral medications used to manage diabetes.
  • Medications used to treat esophageal ulcers and inflammation.
  • Anticholinergic agents used to treat gastrointestinal cramps, urinary bladder spasms, asthma, motion sickness, muscular spasms, Parkinson's disease, and as an adjunct to anesthesia.
  • Cholestyramine, colestipol, and other resins (medications primarily used to manage high lipid levels in the blood).
  • Barbiturates, narcotics (medications with sedative effects), and alcohol.
  • Consult your doctor or pharmacist if you're uncertain about the medications, vitamins, or supplements you're taking and whether they may affect Exforge HCT. Your doctor or pharmacist will have a more comprehensive list of medications to be cautious of while using Exforge HCT.

    How should I store it?

    Exforge HCT should be stored at room temperature. It may be exposed to temperatures between 15°C to 30°C for short periods, such as during transport. Keep it in a cool, dry location, tightly sealed, and protect it from light and moisture.

    Initial signs of overdosage of Exforge HCT are hypotension and tachycardia. Seek medical help immediately.
