Why is Kisqali prescribed?

Kisqali (Ribociclib) is a medication primarily used to treat hormone receptor (HR)-positive, human epidermal growth factor receptor 2 (HER2)-negative breast cancer. It is commonly prescribed for advanced or metastatic cases—where the cancer has spread beyond the breast. Kisqali can also be used for early-stage HR-positive, HER2-negative breast cancer that is confined to the breast or nearby lymph nodes. It is typically taken in combination with another cancer therapy.

What does it do?

Kisqali is a type of medicine known as a kinase inhibitor. It works by blocking specific enzymes called CDK4 and CDK6, which play a role in helping cancer cells grow and divide. By interfering with these enzymes, Kisqali slows down or stops the growth of cancer cells, which can lead to the shrinking of tumors.

How should I take it?

Follow your healthcare doctor’s instructions carefully when taking Kisqali. You can take it with or without food and should be taken each day, if possible, in the morning.

What if I missed a dose?

If you miss a dose of Kisqali or vomit after taking it, do not take an extra dose that day. Simply take your next scheduled dose at the usual time.

What are the special precautions before taking Kisqali?

  • Have signs of infection (fever, chills)
  • Have liver or kidney problems
  • Have uncontrolled high blood pressure
  • Have low thyroid function (hypothyroidism)
  • Have heart issues (heart failure, irregular heartbeat, QT prolongation, history of heart attack, slow heartbeat)
  • Have imbalances in electrolytes like potassium or magnesium
  • Are pregnant, planning to become pregnant, or breastfeeding

Fertility (for males): Kisqali may reduce fertility in males. Speak with your doctor if this is a concern.

Does it have any side effects?

In people with early breast cancer, the most common side effects include:

  • Infections
  • Headache

  • Constipation
  • Hair thinning or hair loss
    Serious side effects may include:
  • Lung Inflammation: Kisqali can cause serious or life-threatening lung inflammation. Contact your healthcare provider if you experience chest pain, shortness of breath, or a persistent cough.
  • Severe Skin Reactions: Seek immediate medical help for symptoms like a worsening rash, blisters on the skin or in the mouth, or skin peeling, with or without fever.
  • Heart Rhythm Issues (QT Prolongation): Kisqali may affect your heart rhythm and lead to a dangerous condition. Your doctor will monitor your heart and run blood tests during treatment. Report any fainting, dizziness, or abnormal heartbeat.
  • Liver Problems: Kisqali can cause serious liver damage. Regular liver tests are required. Notify your doctor if you have upper abdominal pain, yellowing skin/eyes, dark urine, or easy bruising/bleeding.
  • Low White Blood Cell Counts (Neutropenia): This common side effect can increase the risk of serious infections. Your blood cell counts will be monitored. Contact your provider if you develop fever or chills.

Is it safe to take it with another drug?

  • Strong CYP3A inhibitors (e.g., Clarithromycin, Ritonavir, Verapamil, grapefruit): These should be avoided with Kisqali. If they must be used, your Kisqali dose may need to be reduced.
  • CYP3A4 inducers (e.g., Rifampicin, St. John’s Wort, Glucocorticoids): Avoid using these at the same time as Kisqali.
  • CYP3A substrates (e.g., Midazolam, Lovastatin, Simvastatin, Sildenafil): Doses of these drugs may need to be adjusted if taken with Kisqali.

How should I store it?

If you or someone else takes too much Kisqali, seek medical attention immediately.

Infertility: Kisqali may affect fertility in people assigned male at birth. Discuss any concerns with your healthcare provider.

Breastfeeding: Do not breastfeed while taking Kisqali and for at least 3 weeks after the last dose, as it is unclear if the drug passes into breast milk. Let your doctor know if you are breastfeeding or plan to.
